Aug. 12, 2014
Participants interested in trying out for the Bulldogs must be enrolled as a full-time Louisiana Tech student in the upcoming fall quarter and meet all NCAA Clearing House requirements.
Additionally, participants must complete the forms that are linked above and take them to the Louisiana Tech Sports Medicine Department, which is located in the Charles Wyly Athletic Complex, to have a full-time trainer sign off on them before taking part in the tryout.
Those interested in participating in the tryout must also provide the LA Tech trainers the following:
Proof of valid medical insurance that covers up to $90,000. A copy of the front and back of the insurance card will be sufficient.
A copy of a current physical (within the past six months). If the student does not have a current physical, she can call Green Clinic - Northside at 318-255-1515 to set an appointment to get a physical. They are located at 1402 Celebrity Drive.
Proof of Sickle Cell Test - A copy of results from either primary physician at home or physician from Green Clinic.
Participants must then take the completed forms, along with a copy of their insurance card (front and back) and their fall class schedule, to the LA Tech Baseball office, which is located in the Thomas Assembly Center, no later than 1 p.m. on Sept. 4.
